A joint may complain after extra walking, lifting, reaching, or standing. Old injuries or arthritis sometimes leave it stiff and swollen. We’ll begin with what may help.

Why does my joint feel sore?

More activity can leave a joint aching without causing a new injury. Kneeling may bother your knee, while work above your head may tire a shoulder; a hip may stiffen after a long drive.

Arthritis often brings aching, swelling, and stiffness that come and go. An older injury may make busy days harder on the same joint, so that history matters.

A tendon is a sturdy band linking muscle and bone. Repeated motion can make that band or the nearby muscle sore, and the ache may seem to sit inside your joint.

Several stiff or swollen joints can have another cause; inflammation means the body’s defense system is making a joint irritated and swollen. Your doctor will want to know how many minutes the joint stays stiff after waking.

A shorter walk or an easier reach may settle a recent flare, and you can pause the one motion that clearly brings it on. Keep other movement gentle.

Warm water often feels pleasant on a stiff joint. A cold pack wrapped in cloth may soothe puffiness after a busy day. Neither belongs straight against your skin.

  1. A systematic review and meta-analysis of 28 studies covering 266,227 cases of lower limb osteoarthritis quantified occupational load. Lifting heavy loads (more than 10 kg per week) raised the odds of knee OA (OR 1.52, 95% CI 1.29-1.79), as did squatting or kneeling (OR 1.69, 1.15-2.49), standing more than 2 hours daily (OR 1.22, 1.02-1.46) and walking (OR 1.40, 1.14-1.73). A 2025 Bayesian network and dose-response meta-analysis of 92 randomised trials in 6,079 people with knee or hip osteoarthritis found aerobic training most likely to rank first for pain relief (SUCRA 84.7%; SMD -1.00, 95% CrI -1.50 to -0.62), ahead of strength plus flexibility (SUCRA 73.0%), yoga (63.7%), strength alone (55.9%) and flexibility alone (39.8%) - but with NO statistically significant difference between exercise types. Pooled across modalities, the dose-response relationship was U-shaped, meaning more exercise is not linearly better.

A systematic review of 78 prospective primary care cohort studies involving more than 48,000 participants with musculoskeletal conditions identified five factors that predict a worse outcome regardless of which body part hurts: widespread pain, high functional disability, somatisation, high pain intensity and previous episodes of pain. Consistent evidence also showed that use of pain medication was NOT associated with outcome.

In a structured review of 14 studies covering 6,242 patients with an acutely painful swollen joint (653 with confirmed septic arthritis), no single symptom rules the diagnosis in or out: joint pain was present in 85%, a history of joint swelling in 78%, and fever in only 57%. The most powerful bedside data came from aspirating the joint - the summary likelihood ratio rose with the synovial fluid white cell count, from 0.32 below 25,000/microL to 2.9 at 25,000 or more, 7.7 above 50,000 and 28.0 above 100,000.
